where does the form appear?
after the user has been registered or in the registration form?
normally for this kind of requirement, it's like verification document that only admin can view, apart of the uploader
normal contact form sent to the admin doesn't fit you well?
I do understand the once time only but you really need a custom php for that specific purposes
